The Maternal Telehealth Program provides services from the time you enroll until your baby’s first year.

These services are via telehealth, which means that you meet the team from a phone, computer, or tablet without having to go to the clinic.

This program supports moms during pregnancy to reduce complications of birth.

You will have a support team of a Care Coordinator, a Telehealth Nurse, and a Community Health Worker.

-  A Care Coordinator will ask you what you need and how they can help.

-  A Telehealth Nurse will obtain your blood pressure, weight, and review medical needs, and answer questions about your medical care.

- A Community Health Worker will help you navigate the healthcare system and get access to resources within your community.
