Entry: No purchase necessary. You must submit form by 11:59pm ET on September 30, 2018 to be entered to win. One entry per person. One winner will be selected at random. Winner will be drawn on October 1, 2018. To enter without subscribing send an email to contests@mommypoppins.com with the subject line "Philly Reader Survey Contest" and put your full name, address and phone number in the body.

Notification: Winner will be notified by email within 3 days of the end of the contest and will have 72 hours to reply to claim their prize. Make sure Mommy Poppins is added to your address book and prioritized so that our emails are not going into any promos folders and you do not miss winner notifications. After 72 hours the prize will be reallocated to another winner. The winner (first name and last initial) will also be posted on the Mommy Poppins Philadelphia Facebook page once they have been verified.
